After the parent and student sections of these forms are completed, the form must be sent to Whitney Hagins, Science Department Head, Lexington High School, 251 Waltham Street, Lexington, MA 02421 or returned to the science building, room 305, no later than 3:00 PM Friday, March 15, 2013. This is an absolute deadline to allow decisions to be made in time for the school’s scheduling process.
