New Student Registration:

To register a new student you will need to download and complete the following forms:

Parent or Guardian Checklist for Incoming Students (updated March 3, 2012)

Student Registration Form Pre K - 12 Grade (updated March 3, 2012)

Completed Immunizations/ Current Physical Exam Record (Update March 3, 2012) This document contains two standard forms that will need to be completed by your Physician. Please note that Tuberculosis Test Results may also need to be submitted where applicable.

LPS Student Health Information Form (upated March 3, 2012)

Lexington School Census Form

Information Form about Entering Kindergarten Student - This should be completed by parent and preschool teacher.

Signed Records Release Form - For High School students only.
