Please complete and return this dismissal and permission form for each student for the first day of school.  Please note that the dismissal information will be used by the school and classroom teacher.  Additionally, the permission section of this form allows your child to be photographed, videotaped, and included in school directory information. 

 
The reverse side of the form has the student acceptable use policy on it related to student technology use at Fiske and in Lexington Public Schools.
