IMPORTANT REMINDER:  In keeping with the LPS Wellness Policy, candy and other treats in celebration of birthdays and holidays should be enjoyed at home or in the community, and not in school.  Food sharing is discouraged in order to protect those with serious food allergies. 
 
Please call the school nurse with any health questions or concerns related to your child. For printable health forms go to LPS School Nurses webpage and click on "Health Forms".
 
Please notify  the school nurse of any changes in your child's health status.  This might include an injury, illness, a hospitalization, concern for an ill family member, recent loss, or any situation in which your child may need extra support while in school.
 
Important information from the Nurse's Office:
 

All students entering Grade 7 must have received a tetanus booster (Tdap) within the past 5 years and a 2d varicella vaccine (Varivax) or a documented history of chicken pox disease.  This is a Massachusetts state law.


Any student who wishes to play team sports or participate in intramurals MUST have a current physical on file. NO EXCEPTIONS!! This is mandated by the Massachusetts Interscholastic Athletic Association MIAA ).

 
In order to take medication of any kind in school, the student must have on file a Medication Permission Form signed by both prescriber and parent.  The medication may be delivered to the health room and must be in a pharmacy labeled container.
