Parents of 3rd Grade Students:
The Department of Public Health recommends that children have a physical exam by a health care provider every 3-4 years. To comply with this recommendation, LPS require all students in the 3rd grade have an updated physical exam. 
If you have not already done so, please submit a copy of your child's most recent physical exam as soon as possible.
